Dancer/Performer Waiver & Release Form

All dancer/performers must sign this waiver. All performers are required to acknowledge and sign waiver to participate in this event.

Participation Release and Waiver Form for Activity Find the Star Within Showcase

I, the signee as a participating dancer/or legal guardian of participant under the age of 18  understand that participation in the Find the Star Within Showcase as a dance activity, there is a possibility of physical injury. I voluntarily agree, therefore, to assume all risks and responsibilities for any such injury or accident, which might occur to me (or my child) during this activity. I hereby voluntarily agree to waive my rights and that of my heirs and assigns to hold Strides Ballroom LLC, Jennifer Corey, Taylor Westfall, Molly Buirgy, volunteers, or employees from any liability for such damage, loss, injury, or death. I also agree that any personal belongings left in any common area at the Denver Turnverein venue is my responsibility. I waive my rights to hold Strides Ballroom LLC, Jennifer Corey, Taylor Westfall, Molly Buirgy, and the Denver Turnverein from responsibility for lost, missing, or stolen personal belongings.

Film/Video/Photography Release

I hereby grant Strides Ballroom LLC and/or any of the authorized photographers and videographers’ permission to photograph and or film myself or child during the Find the Star Within Showcase. I understand that myself or child’s name, face, likeness, voice, and appearance may be used for the video Find the Star Within presentation, public relations and/or marketing efforts on behalf of Find the Star Within Showcase  (Strides Ballroom LLC) without any monetary or other compensation.

I have read this document and understand its contents, including the fact that by signing this document I may be giving up legal rights to the maximum extent permissible under applicable law. If I am signing this waiver for a minor, I certify that I am the parent or legal guardian and have the right to waive these rights.
